By virtue of my background as a digitally-enabled health care delivery researcher and cancer pain specialist, I am well prepared to contribute to the proposed Achieving Equity through SocioCulturally-informed, Digitally-Enabled Cancer Pain managemeNT (ASCENT) clinical trial. I have provided cancer pain, rehabilitation, and palliative care for over two decades to patient with all types and stages of cancer. My research has included the development, validation, and valuation of scalable models for delivering pain care to vulnerable, disparities-prone, and physically impaired populations. This work has involved the strategic bundling of electronic health record (EHR) clinical decision support (CDS) tools with bidirectional patient portal-based communication to advance evidence-based supportive care (UH3AG067593), and to align care with guideline-endorsed best practices (UM1CA233033). As an active and multiply certified Epic Physician Builder, I have developed and refined patient portal interfaces for clinical and research applications to optimize their acceptability and usability among diverse clinical populations, including digitally disenfranchised subgroups. I have also overseen the effective dissemination of EHR build for validated interventions beyond the Epic EHR to other commercially available EHR platforms, e.g., Cerner, Allscripts, etc. My work has additionally involved the development of novel patient-reported outcome measures (PROMs) and their integration into EHRs to better align care delivery with patients’ needs. Specifically, my team has brought cutting-edge psychometric approaches to the point of care for the efficient and precise identification of patients’ needs spanning symptoms, function, and social determinants of health. My experience in harmonizing CDS functionalities across EHRs has proven critical for advancing multi-site clinical trials. I am an experienced clinical trialist. As the current PI/MPI of two cluster-randomized, stepped wedge, pragmatic trials testing bundled EHR CDS/portal interventions, I have
broadly implemented interventions across diverse sites throughout the Mayo Enterprise, including a high proportion of rurally situated clinics. My team has developed and tested strategies for recruiting and retaining rurally based patients in pragmatic trials, as well as observational and explanatory research. In summary, my experience prepares me to fulfill my role as ASCENT co-PI, particularly in advancing the project’s focus on EHR-enabled approaches to mitigating disparities in cancer pain.
